In many documentaries I see about US soldiers being trained, the drill sarge's hammer on the fact that killing becomes automatically, no thought, just pull the trigger and kill the enemy.

It could be that in certain cases, the wiring goes faulty.

Then an innocent bystander becomes a victim, or enemy. And because the killing became second nature, it could be the case that it just happends.
